As women age, the number of follicles in the ovaries gradually decreases, and hormone secretion patterns also change. During perimenopause, menstrual cycles may become less regular than before. Does having regular periods mean the ovaries are healthy?
Not necessarily.

When evaluating ovarian reserve, doctors generally consider age, AMH, baseline hormone levels, antral follicle count, and other relevant information. AMH and AFC can help doctors assess ovarian reserve and estimate the number of eggs that may potentially be obtained during ovarian stimulation, but these measures alone cannot represent a person's overall reproductive potential. Age remains an important factor in assessing female fertility.
Which menstrual changes deserve particular attention?
If your menstrual cycle was previously stable but has recently undergone persistent changes, such as periods becoming increasingly frequent, intervals between periods becoming significantly longer, a substantial change in menstrual flow, or bleeding between periods, further evaluation may be appropriate.
It is important to note that abnormal menstruation does not necessarily mean ovarian aging. Thyroid disorders, abnormal prolactin levels, uterine fibroids, polyps, adenomyosis, and other gynecological conditions can also cause menstrual changes. Significant or persistent abnormal bleeding should be evaluated by a doctor rather than being attributed directly to age.
